The Conway Medical Center (CMC) Diabetes Self-Management Training Program has recently renewed its status as an accredited diabetes education program by the American Association of Diabetes Educators. This allows residents in and around Horry County increased access to critical diabetes education services.
Diabetes education is a collaborative process through which people with gain the knowledge and skills needed to modify behavior and successfully self-manage the disease and its related conditions. “When you first find out you have diabetes you are likely to have many questions and concerns,” said Mary Jo Hamrick, BSN, RN, CMC Diabetes Nurse Educator. “Our Diabetes Self-Management Training program is designed to give you the tools and information you need to make better lifestyle choices and control your disease.”
The CMC Diabetes Self-Management Training Program offers individual appointments with the Diabetes Nurse Educator, consultations with a dietician, small group classes, and a diabetes support group to educate patients. In addition to these services, the program also provides outpatient diabetes education at Conway Physicians Group (CPG) Family Medicine @ Creel Street and will soon extend these services to other CPG practices to make the program more accessible to patients. Through the Diabetes Self-Management Training Program patients can also meet with an exercise physiologist for an exercise consultation and participate in supervised exercise.
“Trends show that diabetes education is moving out of the hospital and into the community, so AADE’s accreditation program was created, in part, to encourage diabetes education where the patient is seeking care,” said Leslie E. Kolb, RN, BSN, MBA, Program Director, Diabetes Education Accreditation Program. “CMC’s Diabetes Self-Management Training Program is exactly the type of program we envisioned when we set up our accreditation program in 2009.”
